Welcome to the Sixth Star Agreement and Release Form section. As a Sixth Star cruiser, you get access some of Sixth Star’s documents.  Those of you who have traveled with us before have seen these documents in emails that we have sent to you. We have now made it easier for you to access these documents at the click of a button. In case you lose a document you can simply go to our site and download the document. It makes it very convenient to get the documentation that you need from us faster. Below is a list agreement and release forms for each cruise line.

Once filled out in it’s entirety you can then email it to your account manager or fax it to 954-462-0737.
